【Oral Care】The Anti-Cancer Gap: How Oral Mucositis Impacts the True Value of Cancer Drugs

In the course of oncology treatment, attention is often solely focused on the tumor-killing rates of targeted therapy, chemotherapy, or immunotherapy, while a critical point is frequently overlooked: severe drug-induced oral mucositis. According to a large-scale retrospective clinical study by Elting et al., published in Cancer in 2003, 19% of patients with severe oral mucositis were forced to undergo dose reductions, and a staggering 28% experienced treatment delays. This data reveals a stark reality: the failure of many anti-cancer regimens stems not from the inefficacy of the drugs themselves, but from the agony caused by mucosal ulcers. This pain erodes patients' desire to eat and their baseline physical strength, ultimately forcing physicians to interrupt or lower standard treatment doses.

oral mucositis

Faced with this involuntary decline in medical compliance, the 2020 clinical practice guidelines issued by the Multinational Association of Supportive Care in Cancer and the International Society of Oral Oncology (MASCC/ISOO) pointed out that during chemoradiotherapy, patients' immunity is severely compromised. Therefore, the use of topical anti-inflammatory ointments containing steroids is not recommended.

Against this backdrop, the value of physical barrier therapies is being re-examined. These macromolecular medical devices, containing high concentrations of hyaluronic acid or polyvinylpyrrolidone (PVP), function purely through physical isolation. A double-blind clinical trial published in Supportive Care in Cancer by Hadjieva et al. in 2014 demonstrated that in head and neck cancer patients, pain scores decreased significantly within minutes of application. In the same year, Allison et al. published a consistent conclusion in the journal Cancer; the use of macromolecular hydrogels can effectively delay the onset of severe ulcers. These physical films can coat exposed nerve endings, isolate the frictional stimulation of food, saliva, and acidic or alkaline substances, and do not interact with any chemotherapy drugs.

Synthesizing various empirical evidence, physical barrier films can instantly block pain, allowing patients to maintain basic oral intake capabilities. This does more than just help patients endure treatment; these non-pharmacological adjunctive devices are also the pillar ensuring that high-value chemotherapy drugs can be administered at full dose and on schedule according to medical orders. Therefore, only by first securing the patient's mucosal defense line and physical baseline can anti-cancer drugs demonstrate their maximum clinical efficacy without interruption.
